Form Payment Worksheet

The Payment worksheet is used to record the information of the balance due payments of the return or estimated tax payment or amount payable at the time of extension.
You should check the box at the top of the form which is applicable to you. If you are paying estimated tax then should check that box or if you are applying for extension then should check the box for extension and should file Form 4868.

Payment Information The program transfers amount you owe from respective line of the applicable forms. You should enter the RTN no. account number and account type of the taxpayer. You should also enter the amount you are paying and requested payment date.
